About the Role

We are seeking a dedicated and experienced Practice Nurse to join our team at Moss Side Medical Centre. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and contributing to the delivery of our practice's policies and procedures.

Key Responsibilities

  • Conduct patient assessments and provide evidence-based practice nursing care in accordance with our practice's policies and guidelines.
  • Administer vaccinations, treatments, and perform cervical cytology screening, spirometry, and other diagnostic procedures as required.
  • Develop and implement health promotion and wellbeing programs to support patients with chronic diseases, such as diabetes, asthma, and COPD.
  • Collaborate with our interdisciplinary team to optimize patient outcomes and ensure seamless care delivery.
  • Provide routine nursing care to patients, including wound care, medication management, and patient education.
  • Oversee the provision of chronic disease clinics and deliver patient care as necessary, referring patients to secondary or specialist care when required.
  • Implement vaccination programs for adults and support the clinical team with safeguarding matters in accordance with local and national policies.

Person Specification

We are looking for a qualified and experienced Practice Nurse who possesses the following skills and qualifications:

  • Nursing degree and registration with the NMC.
  • Experience working in primary care, preferably in a GP practice.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Strong clinical decision-making skills and the ability to prioritize tasks and manage time effectively.
  • Ability to maintain accurate patient records and ensure confidentiality.
