Our dedicated Community Care Facilitator will become an integral part of our Home First Team, focusing on reablement through a partnership with various care providers.
The reablement strategy is designed to help individuals return to or stay in their residences. We are in search of candidates who possess the capability to inspire individuals, facilitating their recovery, promoting independence, and expediting their return home from healthcare facilities.
As a member of our committed team, you will receive tailored training to foster your growth in this position. It is essential to exhibit strong communication abilities and a compassionate demeanor towards older adults who may experience communication challenges or have complex health conditions.
Our team operates a service seven days a week for community patients, and we are open to discussing flexible working hours and schedules.
Key Responsibilities
The primary objective of this role is to deliver a dynamic and responsive service that maximizes reablement opportunities for patients within the community.
Community Care Facilitators in each locality will contribute to an Integrated Care Pathway for older patients, encompassing a variety of services aimed at preventing hospital admissions and supporting safe discharges.
The successful candidate will be responsible for providing care packages to patients in their residences, which may also extend to residential and nursing facilities. We offer a mileage reimbursement for travel in your personal vehicle and access to company vehicles when available.
Flexibility in work patterns is crucial to meet service demands, and while you will be scheduled to provide necessary services, we are happy to explore flexible options with you.

Qualifications & Experience
Essential
- NVQ Level 3 qualification in care or equivalent experience
- Proven experience in a care environment
- Valid driving license
Desirable
- GCSE Maths & English or equivalent qualifications
- Additional relevant training in healthcare
