Our newly appointed Community Care Facilitator will become an integral part of our Home First Team, focusing on reablement as part of a collaborative effort among community, acute, and social care providers.

The reablement strategy is designed to help individuals return to or stay in their homes. We are in search of candidates who possess the capability to inspire individuals, facilitating their recovery, enhancing their independence, achieving their objectives, and expediting their return home from healthcare facilities.

You will join a committed team and receive tailored training to foster your growth in this position. Strong communication skills and a positive demeanor towards older adults, who may face communication challenges or complex health issues, including frailty, are essential.

Our team operates a service seven days a week for community patients, and we are open to discussing flexible working hours and days.

Key Responsibilities

The primary aim of this role is to deliver a versatile and responsive service that maximizes reablement opportunities for patients within the community.

Community Care Facilitators in each locality will be part of an Integrated Care Pathway for older patients, encompassing a variety of services to avert hospital admissions and support discharges.

The successful candidate will be responsible for providing care packages to patients in their residences, which may also extend to residential and nursing homes. We offer a mileage allowance for travel in your personal vehicle and access to pool cars when available.

Flexibility in working patterns is expected to meet service demands, and you will be scheduled to provide the necessary service; however, we are willing to discuss flexible opportunities with you.

Approximately 568,000 individuals reside in our area, with a significant portion relying on acute hospital services. Consequently, we collaborate closely with our partners to ensure comprehensive care.

Qualifications & Experience

Essential

  • NVQ Level 3 qualification in care or equivalent experience
  • Proven experience in a care environment
  • Valid driving license

Desirable

  • GCSE Maths & English or equivalent qualification
  • Additional relevant training in Healthcare
