About the Role
We are seeking a highly motivated and caring Community Health Nurse to join our team at Cornwall Partnership NHS Foundation Trust. As a Community Health Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in their own homes.
Main Responsibilities
- Assess, plan, implement, and evaluate care for patients, carers, and relatives in their home environment.
- Work autonomously, providing effective, evidence-based care in line with competency and training records, organisational policies, and procedures.
- Support all locality teams by working in a flexible way, moving to work with the team in most need on a daily, weekly, or monthly basis as service provision demands.

Requirements
- Registered Nurse (Adult)
- Evidence of recent relevant professional development
- Ability to act as a mentor for student university placements
- Current Professional Registration - NMC
Desirable Qualifications and Skills
- Hold a post-registration diploma or be working towards a degree
- IT skills to include use of Word, Excel, email, internet, recording systems
- Ability to provide skilled, evidence-based nursing care to a wide range of patients
- Ability to deal with patients and carers who are under stress
- Awareness of and adherence to policies including lone worker and manual handling policies
- Current knowledge of developments in nursing and healthcare in general
