Position Overview

The role of a Patient Care Coordinator at East Trees Health Centre involves regular interaction with patients through both in-person and telephonic communication, facilitating appointment scheduling, and managing follow-up tasks related to patient care.

The primary objective of this position is to assist and guide patients towards the most suitable healthcare services tailored to their individual needs. This includes non-clinical assessment of patient requirements, ensuring they connect with the appropriate healthcare professional at the right moment, which may not always be a general practitioner. This role demands patience and dedication, but it is immensely fulfilling, and the job satisfaction within our team is evident.

This dual-function position (navigation and coding) allows you to support two distinct teams, as you will be engaged in both our reception area and administrative office. This variety not only enriches your work experience but also provides opportunities for personal and professional growth.

Key Responsibilities

The Patient Care Coordinator will engage in daily interactions with patients, both face-to-face and via phone, to manage appointment arrangements and follow-up tasks.

The role focuses on guiding patients to the most appropriate healthcare services based on their needs. This includes non-clinical triage, ensuring patients are directed to the right professional at the right time.

You will be responsible for the effective management of patient inquiries and related administrative tasks. This includes directing patients to other services and communicating on behalf of the clinical team.

We pride ourselves on providing both routine and non-urgent appointments, available in-person or over the phone. Patients are given options, and we strive for continuity in their care. Our practice does not operate a duty doctor or triage system, meaning that the individual who joins our team will engage in a challenging yet diverse role, where personality is key to our friendly environment. We offer training for the right candidate, even if they lack prior experience.

Accurate coding of correspondence from external agencies is a critical aspect of this role, ensuring that patient records are updated with essential clinical information. This task is closely linked to navigation, as you will become familiar with patients and gain valuable medical terminology along the way.

About Our Team

Our team comprises 10 general practitioners (including 4 partners), 4 nurses, 2 healthcare assistants, 3 clinical pharmacists, 2 pharmacy technicians, 3 prescription clerks, 2 dietitians, a care coordinator, and a well-being coach. Our facility also accommodates the community nursing team, health visitors, midwives, podiatry, physiotherapy, speech and language services, bladder and bowel services, eye screening, urology, DVT clinic, FGM clinic, and the Bristol drugs project, among others. We serve a diverse patient population of 16,000 within the inner city.

Qualifications

Essential:

  • GCSE grade A-C in English and Mathematics
  • Strong verbal and written communication skills
  • Proficient in computer use (ability to learn on a PC)
  • Familiarity with applications such as Word, Excel, and email
  • Ability to perform under pressure
  • Capable of working independently as well as collaboratively within a team

Desirable:

  • Experience in a healthcare environment (primary or secondary care)
