About the Role
We are seeking an exceptional individual to join our team as a Discharge and Patient Flow Coordinator. This is a full-time position, working Monday to Friday, 8am to 4pm.
Key Responsibilities
- Act as the first point of contact for internal and external enquiries related to patient discharge.
- Facilitate effective communication between hospital staff, patients, and their families regarding discharge planning.
- Manage patient communications through various channels to create letters and emails.
- Provide support and assistance to clinical and non-clinical staff as needed.
- Liaise with teams across the Trust to ensure seamless communication and coordination.
- Escalate patient queries and issues to the line manager or relevant clinical staff as necessary.
