Discharge and Patient Flow Coordinator

University Hospitals Bristol and Weston NHS Foundation Trust (UHBW) is seeking a highly skilled Discharge and Flow Coordinator to join our Transfer of Care Hub. As a key member of our team, you will play a pivotal role in supporting the effective management of patient discharge from hospital, working collaboratively with ward-based colleagues to ensure seamless transitions to community care.
Main Responsibilities- Act as a first point of contact for internal and external enquiries relating to patient discharge, providing exceptional communication and engagement skills.
- Facilitate and support effective and early communication between hospital staff, patients, and their families regarding discharge planning.
- Manage patient communications via telephone and Trust systems, creating letters and emails as required.
- Liaise with clinical and non-clinical teams across the Trust via telephone and email, escalating patient queries, capacity problems, and general issues to the line manager or relevant clinical staff as necessary.

Person Specification- Essential qualifications: Educated to GCSE, grade C or above (or equivalent) in English Language and Maths, Business and Administration Level 3 NVQ or equivalent experience.
- Desirable qualifications: Previous experience of using a Patient Administration System, previous NHS experience.
- Essential skills: Effective and polite manner when dealing with people, ability to work under pressure and meet deadlines, ability to work effectively in a busy team, adjust to change, and motivate others.
- Desirable skills: Working knowledge of admission policies and procedures.
