About the Role

We are seeking a highly motivated and adaptable individual to join our Discharge to Assess, Triage and Response Team as a Discharge Coordinator. As part of Bristol's Adult Services, you will work with service users, families, and carers who are currently in hospital, supporting their safe and appropriate discharge.

Key Responsibilities

  • Support service users and their families/carers to understand their unique circumstances and priorities, championing the principles of choice and control.
  • Work with individuals to make their own decisions and empower them to have services organised and risks managed in ways that are right for them.
  • Maintain and update information systems and databases to ensure accurate service transactions.
  • Provide advice to customers, contacts, external organisations, and colleagues on policies, procedures, and processes.
