About the Role

We are seeking a highly motivated and experienced Discharge to Assess Coordinator to join our team at Bristol City Council. As a key member of our Adult Services team, you will play a vital role in supporting the discharge of service users from hospital, ensuring they receive the necessary care and support to maintain their independence.

Key Responsibilities

  • Work closely with service users, families, and carers to understand their unique needs and circumstances.
  • Support the development and implementation of personalized care plans, ensuring seamless transitions from hospital to community-based care.
  • Collaborate with internal and external partners to ensure effective communication and coordination of care services.
  • Maintain accurate and up-to-date records, using information systems and databases to ensure efficient service delivery.
  • Provide expert advice and guidance to customers, contacts, and colleagues on policies, procedures, and processes.
