About the Role

We are seeking a highly skilled and dedicated Assistant Practitioner to join our dynamic and caring team at Norfolk & Suffolk Foundation NHS Trust. As an Assistant Practitioner, you will play a vital role in providing patient-centered care and support to individuals with mental health needs.

Key Responsibilities
  • Provide high-quality care and support to service users with mental health needs, working collaboratively with multidisciplinary teams to deliver evidence-based interventions.
  • Contribute to the development and implementation of recovery-focused care plans, ensuring that service users receive timely and effective support.
  • Work closely with colleagues to ensure seamless delivery of services, promoting a culture of collaboration and teamwork.
  • Participate in reflective practice sessions, clinical supervision, and line management supervision to ensure ongoing professional development and growth.
  • Contribute to quality improvement projects within the service, driving innovation and excellence in patient care.

Requirements
  • Care certificate or relevant equivalent.
  • Foundation Degree in Health and Social Care/Mental Health or apprenticeship in H&SC/Mental Health L5.
  • Experience working with service users with mental health needs, within a multidisciplinary team.
  • Working knowledge and understanding of the CPA process.
  • Able to travel independently and possess a flexible attitude.
