Mental Health Discharge Coordinator

We are seeking a highly motivated and experienced Mental Health Discharge Coordinator to join our dynamic team within Inverclyde Mental Health Services. This role offers an exciting opportunity to make a real difference in the lives of patients by ensuring smooth and timely transitions from inpatient care to community settings.
Key Responsibilities:
- Provide direct line management and supervision to a dedicated discharge coordination team, fostering collaboration between health and social care professionals.
- Coordinate patient care plans, facilitating seamless discharges from mental health inpatient beds across the campus.
- Monitor key performance indicators related to discharge activity, ensuring adherence to established targets and best practices.
- Lead clinical governance activities, including care assurance, standards compliance, professional regulation, complaint management, staff governance, and implementation of national and board policies/guidelines.
- Collaborate closely with community and inpatient teams to create a positive discharge experience for patients and their families.
Essential Requirements:
- Registered Mental Health Nurse with the NMC.
- Proven experience as a Band 6 nurse, demonstrating expertise in mental health care.
