Community Care and Treatment Coordinator

Greenock, Inverclyde, United Kingdom NHS Scotland Full time

NHS Greater Glasgow and Clyde stands as one of the largest healthcare systems in the UK, employing approximately 40,000 professionals across a diverse range of clinical and non-clinical roles. Our commitment is to deliver comprehensive acute hospital, primary, community, and mental health services to a population exceeding 1.15 million, extending to 2.2 million when considering our regional and national services.

Overview of the Role:

The Community Treatment and Care (CTAC) services are integral to the Primary Care Improvement Plan (PCIP), established as part of the 2018 GP contract. CTAC facilitates patient access to essential healthcare services that were traditionally managed by general practice. This encompasses vaccination delivery, nurse-led treatment facilities, now referred to as "CTAC centres," and GP-based phlebotomy services. The successful candidate will join the CTAC coordinator team, playing a vital role in the execution of these services.

Key Responsibilities:

The primary focus of this position is to oversee and enhance the delivery of services within Community Treatment and Care (CTAC). Responsibilities include, but are not limited to:

  • Facilitating vaccination delivery.
  • Managing CTAC centres and GP-based clinics within the community.
  • General management duties, including rostering, absence management, and conducting appraisals.
  • Day-to-day management of nursing teams at Band 3 and Band 5 levels.
  • Supporting staff in delivering exceptional patient care within CTAC services.

Candidates must possess strong organizational skills and the ability to manage a dynamic and varied workload. Excellent communication skills are essential for effective collaboration with our multidisciplinary team dedicated to patient care.

Qualifications and Skills:

A first-level registered nurse with significant post-registration experience is essential. While community experience is advantageous, it is not mandatory. As part of the pre-employment verification process, candidates must provide evidence of current NMC registration prior to their start date.

This position requires PVG Scheme Membership / Disclosure Scotland Check. The role involves travel across the designated area to visit patients in their homes, care facilities, and clinics/health centres. Vehicles are available for use if necessary, and applicants should hold a valid driving license. Reasonable adjustments will be considered for candidates requiring them.

Inclusivity Statement:

NHS Greater Glasgow and Clyde actively encourages applications from all community sectors. We are committed to fostering an inclusive culture within our organization and take pride in our diverse workforce.

By endorsing the Armed Forces Covenant, NHS Greater Glasgow and Clyde has affirmed its dedication to being a Forces Friendly Employer, recognizing military skills, experience, and qualifications throughout the recruitment process.
