We are committed to reducing our full-time working week from 37.5 to 37 hours per week, with no change in pay, starting April 2024. This reduction will be applied pro rata for part-time staff.

An Exciting Opportunity Awaits:

We are seeking a highly motivated and experienced individual to join our team as a Discharge Coordinator for Inverclyde Mental Health Services. This role offers the chance to make a real difference in the lives of patients by ensuring smooth and timely discharges from mental health inpatient beds.

Key Responsibilities:

  • Directly manage an integrated discharge coordination team between health and social care within Inverclyde HSCP.
  • Coordinate patient care, focusing on timely discharge from mental health inpatient beds across the mental health campus.
  • Support strategic planning and delivery of this new and developing post.
  • Monitor activity related key performance indicators (KPIs).
  • Lead clinical governance activities, including care assurance, standards compliance, professional regulation, complaint response, staff governance, clinical supervision, line management supervision, legislative frameworks, national and board policy/guidelines implementation.
  • Collaborate with community and inpatient teams to ensure a positive discharge experience for patients and their carers.

Essential Requirements:

Registered Mental Health Nurse with the NMC and robust experience as a Band 6 nurse.
