**Care Coordinator**

**Overview**:

- To manage the arrangement of care support staff provision for service users in their own home.
- To undertake regular monitoring of service in the service user’s home to ensure that a high-quality service is delivered at all times.
- To promote a caring environment for service users through high standards of professional practice which are conducive to the physical, emotional, social, intellectual and spiritual needs of the service users.
- To ensure that each service user receives care appropriate to their individual need.
- Supervise care staff.

**Person Specification**:

- Driving licence (desirable)
- Health & Social care Diploma qualification or care qualification equivalent Diploma level 2 in Adult Care and or working towards level 3 (desirable)
- Sensitivity to the needs of others particularly to the sick or infirm (essential)
- An active team player but also able to work on own initiative (essential)
- A good communicator (essential)
- Commitment to high standards (essential)
- Commitment to training. (essential)
