**Overview**:
We are seeking a dedicated and organised Care Coordinator to join our team. This role is crucial in ensuring that our clients receive excellent quality care and support to keep them safe and comfortable in their own homes. **Key Responsibilities**:
**Coordination and Scheduling**:

- Ensure carers are appropriately rostered and allocated to clients so that care is delivered on time, safely, and in line with clients' wishes as outlined in their care and support plans.
- Handle new client referrals promptly and efficiently.
- Process any changes to clients’ care and support needs.
- Ensure adequate coverage to deliver care in accordance with clients' personal care plans.
- Schedule carers based on their skills, experience, and availability, while considering client preferences, travel arrangements, and working patterns.

**Maintaining Professional Communications**:

- Represent the company professionally in all forms of communication (telephone, face-to-face, and written correspondence).
- Ensure prompt and polite telephone responses.
- Keep the Registered Manager informed of urgent matters.
- Produce reports as required and maintain accurate records.
- Communicate calmly and professionally with clients and their representatives.
- Distribute staff rotas at agreed intervals.
- Engage with clients and their representatives about their care, taking account of different communication needs and levels of understanding.

**Working with Staff**:

- Provide detailed work schedules and instructions/guidance to carers.
- Liaise with the Registered Manager, Director, or recruitment manager to ensure the right number of carers with the appropriate skill mixes are recruited.
- Arrange cover for carers' absence.
- Ensure carers have an adequate work/life balance with appropriate scheduling.

**Maintain and Improve Quality and Compliance**:

- Adhere to the Quality Assurance Policy to provide a quality service to clients.
- Maintain confidentiality and comply with the company’s Confidentiality Policy.
- Be conversant with CQC regulations and legislation governing the service.
- Report any issues regarding legislative requirements to the Registered Manager.
- Ensure accidents and incidents are recorded, reported, and acted upon.

**Work Well as Part of the Walfinch Team**:

- Work flexibly to meet business needs.
- Maintain up-to-date electronic and, where required, handwritten records.
- Carry out general office duties.
- Prepare reports as required by the Registered Manager.
- Take joint responsibility for the retention of care staff.
- Participate in the out-of-hours emergency on-call rota as required, effectively dealing with emergency on-call issues and recording emergency calls.

**Experience and Skills**:

- Computer literate.
- Ability to maintain clear and accurate written records and follow statutory reporting procedures.
- Understanding of confidentiality in relation to home care services and why it is essential.
- Good administrative and IT skills, with high attention to detail and accuracy.
- People management skills.
- Excellent written and verbal communication skills.
- Willingness to follow Walfinch's policies, procedures, and instructions.
- Flexibility and commitment to updating knowledge and skills regularly.

**Desirable**:

- Professional qualification.
- Good geographical knowledge of the local area, including roads, routes, and traffic levels.
- Experience using rostering software.
- Experience as a coordinator.
- Understanding of legislation concerned with care provision.
- Understanding of ethical care principles.
- Full driving license.

**Job Types**: Full-time, Permanent

Pay: £23,000.00-£25,000.00 per year
