HomeDot Care Ltd is a North London based domiciliary care company and we are looking for a skilled and motivated Care Coordinator. As a Care Coordinator, you will be responsible for the allocation and rostering of our fantastic team of care and support workers.

CARE COORDINATOR JOB DESCRIPTION HomeDot Care 2021

Care Coordinator reports to Care Manager or Registered Manager Summary of role To ensure all client visits are allocated and to ensure that suitable care workers are matched to clients and to maintain appropriate office procedures.

Principal responsibilities
- Answering all incoming telephone enquiries
- Undertaking care worker supervisions and appraisals
- Partaking in care worker staff meetings
- Processing new client referrals and inputting the support plans on Careplanner and on the PASS system
- Preparing appropriate documentation for Field Care Supervisors
- Permanent allocation/scheduling of care workers to all client visits and ensuring all visits are always covered - ensuring that a 92% compliance percentage is attained with scheduling of care worker visits
- Ensuring that all clients and care worker documentation and electronic records are kept up to date at all times
- Ensuring all other records are kept up to date
- Being fully conversant with Careplanner and the PASS system and actively monitoring and managing the logging in and out of care workers
- To work closely with the Field Care Supervisor to manage referrals and start new care packages efficiently and in a timely manner
- To ensure compliance with all office systems and procedures
- Maintaining of a current spreadsheet which depicts all clients whose schedule needs to be worked on
- Undertaking the on-call service - this will normally work to one week a month but may vary depending on staff capacity
- Being able to cover care visits if no care workers available
