Sciensus are currently looking for a Clinical Transfer Coordinator Watford. We are looking for someone who will be an on-site expert, taking charge for ensuring the smooth and timely discharge of patients. As a Clinical Transfer Coordinator, you will take responsibility for assessing the suitability of patients and guiding them through the journey from hospital, back home. It is our aim to expedite patient’s recovery, whilst maintaining the highest standard of care.

You will be responsible for meeting capacity and contractual targets in a timely manner, whilst always maintaining high degrees of safety and delivering the highest standard of care. Completing clinical assessments, determining patient’s suitability, compiling care pathways and then transitioning care to the wider healthcare at home multi-disciplinary team. Establishing and maintain exceptional relationships, internally and externally with both stakeholders and patients to improve performance and assist the growth of the business. Accurately documenting all patient care and clinical information to provide real time data and clinical history to ensure quality care is maintained and delivered. Maintain and demonstrate up to date knowledge, ensuring any gaps in clinical competency are addressed whilst also staying compliant with all business policies and procedures.
