Position Overview
The primary objective of this role is to join a dedicated team of experienced nurses responsible for the clinical management of care for patients and families referred to our organization.
Key Responsibilities
- Serve as the main contact for patients, families, clients, and healthcare professionals reaching out to the hospice, addressing all clinical inquiries.
- Provide clinical supervision and triage for referrals to ensure timely access to appropriate services for patients and their families.
- Enhance the nursing role within the Coordination Centre to ensure a responsive, evidence-based, and high-quality service for beneficiaries and professionals.
- Engage in monthly rotations as a Specialist Palliative Care Clinical Nurse in various clinical settings to maintain and develop professional skills.
- Actively promote our services to external health and social care professionals and the broader community.
- Conduct audits and case reviews to enhance the service provided by the Coordination Centre.
- Contribute to the strategic development of the hospice, translating local and national strategies related to hospice care to implement effective services.
- Represent the organization as an ambassador, exemplifying our values and positively advocating for the specialized care we deliver.

We support individuals of all ages affected by progressive life-limiting illnesses and their caregivers, ensuring they live as fully as possible by providing accessible palliative care and bereavement services. Our dedicated team offers care for adults and children diagnosed with cancer and other life-limiting conditions, visiting them day and night. This vital work requires talented and passionate individuals.
Job Duties
- Exhibit clinical excellence and uphold consistently high standards of nursing practice in palliative care.
- Serve as a role model, demonstrating exceptional care standards and providing clinical leadership to colleagues.
- Advise and support patients with complex needs and their families at home, addressing physical, psychological, social, and spiritual aspects of care in alignment with our mission and values.
- Mentor new staff and oversee their successful completion and timely sign-off of baseline competencies.
- Conduct performance reviews for direct reports and develop personal development plans.
- Facilitate the dissemination of information through appropriate channels.
- Participate actively in the orientation and mentorship of new staff members.
- Supervise and educate trained staff.
- Provide specialized palliative care training to multidisciplinary team members and nursing home staff within the community.
Qualifications
Essential
- Registered Nurse.
- Degree-level education or modules in Palliative Care or a related field.
Desirable
- Nurse Prescriber or commitment to achieve this qualification.
- Advanced assessment skills training or commitment to undertake this training within one year of appointment.
- Advanced communication skills training.
Experience
Essential
- Demonstrated experience in palliative care.
- Proven experience in managing teams and strong clinical leadership.
- Effective team collaboration experience.
Desirable
- Experience in teaching within a clinical environment.
- Experience in successfully implementing and supporting change initiatives.
