Job Summary

Rennie Grove Peace Hospice Care is seeking a skilled Clinical Coordination Specialist to join our team. As a key member of our small team of senior nurses, you will be responsible for the clinical coordination of care for patients and families referred to our hospice.

Key Responsibilities

  • Provide clinical oversight and triage for referrals to ensure patients and family members receive the right service in a timely manner.
  • Develop the Coordination Centre's nursing role to provide a responsive, evidence-based, and high-quality service to beneficiaries and professionals.
  • Work as a Specialist Palliative Care Clinical Nurse Specialist in a different Rennie Grove Peace clinical setting on a monthly basis to maintain and develop skills.
  • Act as a Rennie Grove Peace Hospice Care ambassador, demonstrating our values and promoting the specialist care we provide.
  • Contribute to the wider strategic development of the hospice and help implement and deliver an effective service.

Requirements

  • Registered Nurse
  • Degree Level or modules in Palliative Care or another relevant subject
  • Proven palliative care experience
  • IT literate, including a good knowledge of Excel, Word, and a clinical IT system
  • Advanced assessment skills and specialist symptom control knowledge
  • Full, current valid UK driving Licence
