Job summary

The purpose of this role is to be part of a small team of senior nurses who are responsible for the clinical coordination of care for patients and families referred to Rennie Grove Peace.

Main duties of the job

Being the point of contact for patients, families, clients and professionals who contact the hospice, dealing with all clinical enquiries. Providing clinical oversight and triage for referrals to ensure that patients and family members get the right Rennie Grove Peace service in a timely manner. Developing the Coordination Centre's nursing role to ensure it provides a responsive, evidence based and high quality service to beneficiaries and professionals. Working as a Specialist Palliative Care Clinical Nurse Specialist in a different Rennie Grove Peace clinical setting on a monthly basis to maintain and develop skills. The post holder will play an active part in promoting Rennie Grove Peace services to others including external Health and Social Care professionals and wider members of the public. The post holder will undertake activities such as audits and case reviews in order to improve the Coordination Centre service. Contributing to the wider strategic development of the hospice and help to translate local and national strategy relating to hospice care and enable Rennie Grove Peace to implement and deliver an effective service. Acting as a Rennie Grove Peace Hospice Care ambassador, actively demonstrating our values and positively promoting the specialist care we provide.

Job responsibilities

Demonstrate clinical excellence and maintain consistently high standards of nursing practice in palliative care. Act as a role model demonstrating high standards of care and providing clinical leadership for colleagues. Advise and support on patients with complex needs and their families at home, maintaining an approach that incorporates physical, psychological, social and spiritual aspects of care, in accordance with the Rennie Grove Peace mission and values. Act as a mentor for new staff and oversee successful completion and timely sign off of their baseline competencies. Carry out individual performance review process (annual and mid-year reviews) for any direct reports and complete personal development plans. Receive and disseminate information through appropriate channels. Participate fully in the orientation / mentorship of new staff. Supervise and teach Rennie Grove Peace trained staff. Provide specialist palliative care training to multi-disciplinary team colleagues and nursing home colleagues within the community. Person Specification

Qualifications

Essential

Registered Nurse. Degree level or modules in Palliative Care or another relevant subject.

Desirable

Nurse Prescriber / commitment to successfully achieve this qualification. Advanced assessment skills training / commitment to attend this training within one year of being appointed to the position. Advanced communication skills training.

Experience

Essential

Proven palliative care experience. Proven experience of managing people / strong clinical leadership. Proven experience of effective team working.

Desirable

Experience of teaching within a clinical setting. Experience of successfully implementing and supporting change.

Additional Criteria

Essential

IT literate including a good knowledge of Excel, Word, a clinical IT system. Excellent written and verbal communication skills. Good organisational skills. Teaching and presentation skills. Advanced assessment skills and specialist symptom control knowledge to plan, implement and evaluate nursing care. Drive and enthusiasm with a positive approach. Caring and empathetic approach.
