Job summary

The purpose of this role is to be part of a small team of senior nurses who are responsible for the clinical coordination of care for patients and families referred to Rennie Grove Peace. We are recruiting for one part time ( hours) and one full time position ( hours)

Main duties of the job

Being the point of contact for patients, families, clients and professionals who contact the hospice, dealing with all clinical enquiries.Providing clinical oversight and triage for referrals to ensure that patients and family members get the right Rennie Grove Peace service in a timely manner.Developing the Coordination Centre's nursing role to ensure it provides a responsive, evidence based and high quality service to beneficiaries and professionals.Working as a Specialist Palliative Care Clinical Nurse Specialist (CNS) in a different Rennie Grove Peace clinical setting on a monthly basis to maintain and develop skills.The post holder will play an active part in promoting the Rennie Grove Peace service to others including external Health and Social Care professionals and wider members of the public.The post holder will undertake activities such as audits and case review in order to improve the Coordination Centre service.Contributing to the wider strategic development of the hospice and help to translate local and national strategy relating to hospice care and enable Rennie Grove Peace to implement and deliver an effective service.Acting as a Rennie Grove Peace Hospice Care ambassador, actively demonstrating our values and positively promoting the specialist care we provide.

Job responsibilities

Demonstrate clinical excellence and maintain consistently high standards of nursing practice in palliative care.Act as a role model demonstrating high standards of care and providing clinical leadership for colleagues.Advise and support on patients with complex needs and their families at home, maintaining an approach that incorporates physical, psychological, social and spiritual aspects of care, in accordance with the Rennie Grove Peace mission and values.Effectively allocate and support team members to implement the care plan, monitoring and evaluating the care provided, and deliver practical nursing care when appropriate.Ensure all written and verbal communication is constructive, effective, accurate and appropriate.Be responsible for developing and maintaining effective working relationships with community professionals, support services and members of other hospices and palliative care teams, providing clinical expertise, specialist advice and support as requested.Demonstrate a research based approach to nursing and participate in on-going Rennie Grove Peace research.Identify risk and lead risk management involved in patient care and lone working, implementing learning outcomes from recorded incidents.Be responsible for identifying and acting on any risk of safeguarding to patients and their families in accordance with the Rennie Grove Peace Safeguarding Policies and Procedures.Participate in setting standards and the audit of patient and family care.Regularly attend and contribute to team, hospice, regular locality multi-disciplinary team and locality meetings, recognising the importance of peer support.Follow Rennie Grove Peace procedures and statutory regulations relating to custody and administration of drugs.Work to structured shift patterns (early, day and late shifts) on a rota basis from Monday to Sunday.Promote equality and value diversity.
