The Patient Pathway Coordinator will lead on tracking the pathways of patients. By working closely with a wide range of administration, clinical and managerial staff, and providing a high standard of administrative support, the post holder will ensure patients receive treatment as soon possible and ensure national targets are met.

The post holder is responsible for providing a high standard of professional and patient focused service to a range of Trust-wide specialities and will support the function on a day-to-day operational basis.
- To be responsible for checking patients into clinics and confirming personal details whilst ensuring patient confidentiality at all times. Including the maintenance of Health Records and Information Management.
- To communicate tactfully with patients and deal with enquiries where there are barriers of understanding.
- Plan and organise multi-disciplinary team meetings.
- Work within a multi-disciplinary team in developing the service in line with departmental plans and Trust corporate objectives.
- To deal with patient queries over the telephone and face to face constantly throughout the day.
- Attend relevant meeting and complete actions regarding the care of the patients within the designated pathway.
- To deal with both patients and relatives queries over the telephone and face to face on a daily basis.
- To ensure that knowledge and process of Chemotherapy and Radiotherapy regimes are fully known and kept up-to-date to ensure that the rotation between the Private Patients and Patient Affairs/Cash Office duties runs smoothly.
- Assist in recording and invoicing Private Patients or other NHS on the Oracle System. This includes keeping the private patient spreadsheet up to date and accurate

ENHT is seeking to maintain and expand the Mount Vernon Cancer Centre staff as we move to an exciting new future.

Main role requirements are below but for the full details please refer to attached a person specification.
- To ensure the completion of private patient forms, completing and signing and witnessing these when the patients attends each appointment.
- To be responsible for ensuring private patient forms are attached to the case notes for completion where appropriate.
- To be responsible for ensuring all self-funding patients either produce a receipt from Patient Affairs confirming payment prior to commencing treatment or attending their appointment or process the payment in the Hal Gray Unit using the Card Net Payment Machine.
- Inform all the relevant areas and consultants that the patient has arrived.
- Ensure all agreement to pay forms are completed and processed correctly as per policy.
- Ensure that all policy and procedures for the Hal Gray Unit are followed and logged at all times.
- Ensure that all notes are in the Hal Gray unit ready for the patient’s appointment and returned to the relevant secretary or filing at the end of the day.
- Ensure all the refreshments are replenished on a regular basis.
- Ensure that the patient area is kept clean and tidy at all times.
- Ensure that all supplies for the Hal Gray Unit are fully stocked and ordered in ample time.
- To support the Private and Overseas Patients, Patient Affairs and Income Team as required.
- To undertake any other reasonable duties as required
