Harrow Health CIC began in 2007 by a group of Harrow GPs, whose sole aim was to ensure patients receive a high-quality clinical care service within the community. Over the years, HHCIC has expanded. We now provide a large number of clinical services, which include East Harrow Walk in Centre, Community Services, and our Whole Systems Integrated Care Services.

**Job Summary**:
We are recruiting for a part time Community Coordinator to join our Community Support Team. The post holder will be responsible for the day-to-day administrative duties and running the community specialist clinics at various sites across Harrow.
Based at our office in Northwood, you will take responsibility for effective managing patient’s bookings, the triage process and using the E-referral service. This is a busy role, with lots of time spent on the phone liaising with patients and booking their appointments. You will need to have a proactive approach and be happy to pitch in and support other services as well as your own, depending on the business requirements.
- Responsible for the day-to-day administrative duties of HHCIC clinics (I.E Musculo Skeletal. Ophthalmology, Paediatrics.
- Managing patient’s bookings, triage process and using e-referral service and provide secretarial support.
- Assist in the preparation of consultant led clinics, ensuring efficient administrative support is provided to facilitate the smooth running of clinics by processing referrals received promptly, booking patients into suitable appointments, and ensuring that all record is maintained.
- Work access various location such as the GP Access Centre, Head Office, and Community Clinics.
- Deal with sensitive and difficult calls effectively and escalate issues to the service manager as appropriate.
- Assist in the collection of activity and audit data to support monthly reporting and invoicing.
- Ensure services meet their KPIs
- Attend various locations such as the Walk in centre and Clinics.
- The role will involve call handling and back office support functions
- To undertake other administrative tasks as required for other clinical services which may commence/ be developed.
- General office duties, to include answering telephone; taking messages; dealing with post; photocopying; faxing and organising meetings as required
- Liaise with patients, clinicians and other service providers such as GP practices, GPwSI’s, consultants and other private organisations working with HHCIC, as required, and providing excellent customer services
- Using E-Referral System and EMIS Web
- Support and provide feedback to Senior Coordinators/Service Manager
**Additional Information**:
**Appraisal**
HHCIC operates a system of individual performance review/appraisal for the purpose of agreeing performance objectives and discussing development needs in line with requirements of service need in the operational plan.
**Clinical Governance**
To have responsibility for a commitment to maintaining a high quality service to patients by continual development of Service in the light of research evidence and by audit, based against clinical relevant standards.
**Code of Conduct for Professional Group**
All members of staff are required to work in accordance with their professional group’s code of conduct (e.g. NMC, GMC, HPC).
**Conflict of Interests**
You may not without the consent of HHCIC engage in any outside employment. You must declare to your manager all private interests, which could potentially result in personal gain as a consequence of your employment position in HHCIC. Interests that might appear to be in conflict should also be declared.

It is a requirement of this position that a Criminal Records Bureau disclosure at the enhanced level is undertaken.
