Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Holderness Health. As a Clinical Pharmacist, you will play a key role in optimising medicines use across our primary care network.

Key Responsibilities

  1. Patient Facing Clinics: Conduct clinical medication reviews with patients, identifying opportunities for medicines optimisation and providing recommendations to senior pharmacists, nurses, and GPs.
  2. Medicines Optimisation: Work with patients to ensure they are taking their medicines safely and effectively, and provide education and support to help them manage their conditions.
  3. Medicines Information: Provide accurate and up-to-date medicines information to patients, GPs, and other healthcare professionals, ensuring that they have the knowledge they need to make informed decisions about medicines.
  4. Medicines Safety: Identify and mitigate potential medicines safety risks, working closely with the practice team to implement changes and improvements.
  5. Service Development: Contribute to the development and implementation of new services that have a medicines component, ensuring that they are safe, effective, and meet the needs of patients.
  6. Education and Training: Provide education and training to primary healthcare teams on medicines optimisation and therapeutics, ensuring that they have the knowledge and skills they need to provide high-quality care.
  7. Quality Improvement: Lead and contribute to quality improvement initiatives, using data and evidence to drive improvements in medicines use and patient outcomes.
