About the Role

We are seeking an experienced Clinical Pharmacist to lead our efforts in geriatric care. This is a part-time position, working 2 days per week on Mondays and Tuesdays.

The ideal candidate will have a strong background in clinical pharmacy, with experience of working in elderly care settings. You will be responsible for providing expert clinical advice to patients and healthcare professionals, as well as conducting medication reviews and implementing changes to improve patient outcomes.

As a member of our multidisciplinary team, you will work closely with doctors, specialist nurses, pharmacy technicians, and therapists to provide comprehensive geriatric assessments and develop individualized care plans.

You must have a Master's degree in Pharmacy (MPharm) or equivalent, plus one year's pre-registration training, and be registered with the GPhC. Post-graduate training in clinical or pharmacy practice to diploma level is also desirable.

Key Responsibilities
  • Lead the development and implementation of policies and procedures related to medicines management
  • Provide expert clinical advice to patients and healthcare professionals
  • Work collaboratively with multidisciplinary teams to develop individualized care plans
  • Conduct medication reviews and implement changes to improve patient outcomes
What We Offer
