Job Summary

Holderness Health is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a vital role in optimising patient care by ensuring the safe and effective use of medicines.

Key Responsibilities

  1. Patient Facing Clinics: Conduct clinical medication reviews with patients, identifying areas for improvement and providing recommendations for senior clinical pharmacists, nurses, and GPs.
  2. Medicines Optimisation: Work with patients to optimise their medication regimens, ensuring they receive the best possible care.
  3. Medicines Information: Provide accurate and up-to-date information on medicines to patients, GPs, and other healthcare professionals.
  4. Unplanned Hospital Admissions: Identify and address the root causes of unplanned hospital admissions, working to reduce the prescribing of high-risk medicines.
  5. Medicines at Discharge: Reconcile medicines following discharge from hospital, intermediate care, and into care homes, ensuring patients receive the medicines they need.
  6. Signposting: Refer patients to the appropriate healthcare professional for the level of care required.
  7. Repeat Prescribing: Manage the repeat prescribing reauthorisation process, reviewing patient requests and ensuring patients have appropriate monitoring tests in place.
  8. Risk Stratification: Identify cohorts of patients at high risk of harm from medicines, working to mitigate these risks.
  9. Service Development: Contribute to the development and implementation of new services with medicinal components.
  10. Information Management: Analyse and present medicines data to support decision-making.
  11. Medicines Quality Improvement: Undertake clinical audits of prescribing, feeding back results and implementing changes.
  12. Medicines Safety: Implement changes to medicines resulting from MHRA alerts and other guidance.
  13. Local and National Guidelines: Monitor practice prescribing against local health economy RAG lists and make recommendations for GPs.
  14. Education and Training: Provide education and training to primary healthcare teams on therapeutics and medicines optimisation.
  15. Care Quality Commission: Work with the general practice team to ensure compliance with CQC standards where medicines are involved.
  16. Public Health: Support public health campaigns and provide specialist knowledge on public health programmes.
