Job summary

Building on Haxby Groups experience as a regional workforceand training hub alongside a vision to become an at-scale multiprofessionalprimary care training provider, this innovative role will allow qualifiedAdvanced Clinical Practitioners with an interest in education to maintain and develop existingclinical skills in a supportive practice environment alongside an expanding educationalportfolio. A proportion of the successfulcandidate's week will be spent working as part of the Haxby Group Trainingteam. This will involve the development and deliveryof teaching and leadership skills, helping to mentor and support both currentand aspiring practice nurse and other health professionals.

Main duties of the job

In the clinical part of the role you will be responsible for delivering quality holistic careand treatment to the Practice population. Whilst you are a dependent practitioner, youare able to work independently and make independent decisions enabled by acollaborative and supportive working relationship with your clinical supervisor. You willdemonstrate safe and effective clinical decision making and expert care, includingassessment, diagnostic and management skills. As well as dealing with acute illness,you may be involved in the management of long-term conditions.

You will be responsible for supporting, designing and delivering training programs thatwill meet the needs of our customers. You will be responsible for performing trainingneeds assessments, designing and delivering curricula and managing all phases oftraining interventions. You will support in the development of education and trainingopportunities, projects and schemes within the remit of Haxby Group Training and thePrimary Care Workforce and Training Hub (PCWTH), liaising with practices acrossHumber and North Yorkshire to provide support and guidance relating to students andtraining. You will be involved in directly supporting and developing other trainers withinthe wider training company to aid deliverance of multiple trainer programs accounting forour wider trainers offer. You will be involved in supervising other clinicians within ourlocal and regional training network.

Job responsibilities

Clinical Responsibilities:

Assess, diagnose, plan, implement and evaluate treatment/interventions and carefor patients presenting with an undifferentiated diagnosis within your sphere ofcompetence Clinically examine and assess patient needs from a physiological andpsychological perspective, and plan clinical care accordingly Assess, diagnose, plan, implement and evaluate interventions/treatments forpatients with complex needs Utilise your clinical supervisor or mentor and other clinical staff appropriately toensure safe and effective patient care, demonstrating awareness of your ownlimitations Primary Care Provide safe, evidence-based, cost-effective, individualised patient care within thesurgery, patients own home or other environment where patient care is carriedout. Refer patients directly to other services or agencies as appropriate, utilising Practiceand local guidelines and in a timely manner Pro-actively identify, diagnose, monitor and manage treatment plans for patientsat risk of developing a long-term condition as appropriate Diagnose and manage acute conditions, integrating both drug- and non-drugbased treatment methods into a management plan Prescribe and review medication for therapeutic effectiveness, appropriate topatient needs and in accordance with evidence-based practice, and national andpractice protocols, and within own scope of practice. Utilise Patient GroupDirections for the supply of medication as appropriate. Work with patients in order to support compliance with and adherence toprescribed treatments. Provide information and advice on prescribed and over-the-counter medication onmedication regimens, side-effects and interactions Prioritise health problems and intervene appropriately to assist the patient incomplex, urgent or emergency situations, including initiation of effectiveemergency care Support patients to adopt health promotion strategies that promote healthy livingand to apply principles of self-care Recognise, assess and refer patients with mental health needs as appropriate Communicate with and support patients receiving bad news

Teaching and Training Responsibilities:

Pro-actively working, leading when required, to support and develop the PrimaryCare Workforce and Training Hub (PCWTH) hosted by Haxby Group. Support in the development of education and training opportunities, projects andschemes within the remit of Haxby Group Training and the PCWTH Supporting with Practice Learning Facilitator work, liaising with practices andeducation providers across Humber and North Yorkshire to provide support andguidance relating to students and training. Identifying and assessing future and current training opportunities and needs. Translating requirements into training services and products that meetopportunities and customer needs. Maintaining a keen understanding of training trends, developments and bestpractice. Developing a training services plan and assist in building the annual trainingdelivery programme. Train the trainers role: directly support and aid development of other trainers Preparing or overseeing teaching plans. Delivering training and courses, deploying a wide variety of training methodsdelivering content in keeping with relevant local and national evidence andguidance. Monitoring and evaluating training programmes effectiveness, success and returnon investment and reporting on them. Identifying and assessing future and current training needs through job analysis,career paths, annual performance appraisals and consultation with line managers. Maintaining a keen understanding of training trends, developments and bestpractice. Assist with or lead on projects as appropriate to develop new businessopportunities regionally, nationally and internationally. Fostering the development of inter-professional learning and the integration ofstudents and trainees into the workplace. Liaising with other members of the Haxby Group Training team and key HaxbyGroup staff to ensure high quality educational provision. Being aware of key performance requirements and targets regarding PCWTH andother training commitments and assist in ensuring they are met in a timelymanner. Developing and maintaining key links between Haxby Group Training and otherstakeholders such as educational establishments or organisations, NHSorganisational bodies, non-NHS establishments, other practices, actual andpotential business clients, and other providers. Attending meetings and collaborative groups with external stakeholders asrequired. Maintaining suitable records of communications, reports and other activities,ensuring others have appropriate access as required, utilising InformationTechnology to maximum benefit. Maintaining a keen understanding of training trends, developments and bestpractice. Delegating work as appropriate. Undertaking additional tasks as required within your role. Knowledge/Qualifications/Skills

Essential

Registered Nurse (NMC) or Registered Paramedic (HCPC) or Registered Physiotherapist (HCPC). Masters Degree in Advanced Clinical Practice. Minor illness qualification at degree level assessed by OSCEs. Minor injuries qualification. Current registration. Independent non-medical prescriber or working towards it or proven ability to use Patient Group Directions. Aware of accountability of own role and that required of a Registered Practitioner. Good communication (oral and written) and inter personal skills. Change management skills and ability to support patients to change lifestyle. Negotiation and conflict management skills. Keyboard skills.

Desirable

At least two years experience post MSc qualification. Mentor/teaching qualification. Knowledge of health promotion strategies. Knowledge of Quality and Outcomes Framework. Completed Train the Trainers training.
