We have an exciting opportunity for an enthusiastic, experienced anddedicated Practice Nurse to join our GP Practice and work as part of our team, alongsideour doctors, paramedics, ANP, Pharmacists, Health Care Assistants and GPAssistant and Admin Roles.

The practice nurse will be responsible for a number ofclinical areas such as infection prevention and control, health promotion,chronic disease management, health prevention, well women and well man clinics,as well as supporting the practice management team in the reviewing anddelivery of clinical policy and procedure.

Main duties of the job

The Practice Nurse will be responsible for a number ofclinical areas such as Cervical smears, Childhood Immunisations, Health Promotion, Health Prevention, Chronic Disease Management, Well Women and Well man clinics, as well assupporting the management team in the reviewing of clinical policy andprocedure.

The successful candidate will be working in conjunction with skilled andexperienced Nurse Practitioner, Practice Nurse, HCAs, Paramedic, PrescribingPharmacist& supported by our GP's.

Job Summary:

We have an exciting opportunity for an enthusiastic, experienced and dedicated Practice Nurse to join our GP Practice and work as part of our team, alongside our doctors, paramedics, ANP, Pharmacists, Health Care Assistants and GP Assistant.

We are looking for a Part time Practice Nurse - hours negotiable within 16-20 hours per week.

The successful candidate will work as part of the primary healthcare team providing Personal Medical Services to the patients of practice

Access to NHS Pension

Up to 33 days paid leave per year (Pro-Rata)

Job Responsibilities:

Providing assessment, screening and treatment services and health education advice

Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols.

Provide general and specific health screenings to the Practice patients (within agreed protocols) with referral to general practitioners as necessary.

Advise patients on general health care and minor ailments with referral to GPs as necessary.

Routine tasks may include, but are not limited to:

o Chronic disease management and review (including diabetes, hypertension, CKD, dementia, asthma, COPD, cancer, palliative , mental health etc)

Disease prevention and screening (cervical smears, smoking cessation, immunisations and vaccines, obesity, alcohol, etc. Other nursing procedures (health measurements etc)

Supplies and equipment Treatment room and other areas

Ensure the maintenance of equipment and stock levels relating to patient care.

Be responsible for cold chain maintenance and vaccine storage including stock rotation etc.

Ensure that the clinical rooms are left clean and tidy and waste is taken away according to practice policy

Pathological specimens and investigatory procedures

Following appropriate training, undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc and ensure the cleanliness of any equipment so used. Perform any investigatory procedures requested by the GPs

Administrative and professional responsibilities

Participate in the administrative and professional responsibilities of the Practice team.

Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice.

Ensure collection and maintenance of statistical information required for regular and ad hoc reports.

Attend and participate in Practice meetings as required.

Assist in the formulation of Practice philosophy, strategy and policy and develop appropriate protocols.

Research projects

Co-operate and participate as required in any research projects within the Practice.

Student/staff education and training

Participate in the education and training of others of all disciplines and the induction of all members of Practice staff with special responsibility for nursing staff.

Liaison

Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the Practice, with appropriate regard to confidentiality.

Professional development

Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.

Qualifications

Essential

Registered first level nurse Membership of a professional body Evidence of CPD and training Qualification or able to evidence experience in the management of diabetes, respiratory, family planning, wound care, cervical cytology

Desirable

Post graduate diploma or degree (Chronic Disease Management) Diploma in Asthma/COPD/diabetes

Experience working within a GP Practice or Primary Care Team Experience of chronic disease management Immunisation and vaccine experience Ability of working independently within boundaries Ability to use own judgement, resourcefulness and common sense. Ability to work without direct supervision and determine own workload priorities.
