As an Advanced Practice Nurse at Modality Partnership Hull Division, you will play a vital role in delivering high-quality patient care and assisting in various medical procedures. This position is ideal for individuals who are passionate about helping others and have a strong knowledge of anatomy.

We are seeking a skilled and compassionate General Practice Nurse to join our healthcare team, working across both West Hull Health Hub and Alexandra Road. You will be responsible for the delivery of evidence-based practice for patients presenting with chronic conditions, and providing preventative health care to the practice population.

This job requires a professional approach to work, good telephone manner, and strong team player skills. As a fully qualified nurse, you should have experience in managing patients with chronic conditions, accountability of own role and other roles in primary care, and knowledge of health promotion strategies.

The estimated salary for this role is £35,000 - £45,000 per annum, depending on experience. Additionally, you will benefit from enrolment to the NHS pension scheme, annual leave minimum 27 days, plus 8 days bank holiday pro rata, employee discounts and benefits scheme, employee assistance programme (EAP), education and career pathways, enhanced family friendly policies, wellbeing support and initiative.
