Reablement Support Specialist

**Job Overview:**

We're looking for a talented Reablement Support Specialist to join our team at Wiltshire Reablement. As a key member of our care team, you'll work closely with occupational therapists to design and deliver personalized reablement plans for customers in their own homes.

Key Responsibilities:

  • Work collaboratively with occupational therapy staff to develop and implement reablement support plans that meet customer needs and regulatory requirements.
  • Ensure effective communication and collaboration between professionals, customers, and their families to achieve desired outcomes.
  • Maintain accurate records and reports on reablement support delivery, identifying areas for improvement and implementing changes as needed.

Requirements and Qualifications:

  • NVQ Level 4 or equivalent relevant experience (or a commitment to pursue one).
  • Excellent ICT skills, including Microsoft applications and specialist systems.
  • Able to conduct client risk assessments and identify eligibility for service provision.
  • HSE recognized First Aid at Work qualification or willingness to achieve one.
  • Prior experience contributing to supervision or ability to demonstrate required skills.
  • Knowledge of relevant legislation and guidance in relation to working with and safeguarding adults.
