Senior Support Worker

**Job Description:**
We are seeking a compassionate and skilled Senior Support Worker to join our team at Wiltshire Reablement. As a core member of our care team, you will work closely with occupational therapists to develop and implement personalized reablement plans for customers in their own homes.
About the Job:
- Work collaboratively with occupational therapy staff to create and deliver reablement support plans that meet customer needs and regulatory requirements.
- Maintain effective communication and collaboration between professionals, customers, and their families to achieve desired outcomes.
- Monitor and evaluate reablement support delivery, identifying areas for improvement and implementing changes as needed.
Requirements:
- NVQ Level 4 or equivalent significant experience in a relevant field (or a commitment to pursue one).
- Proven ICT skills, including Microsoft applications and specialist systems.
- Able to conduct client risk assessments and identify eligibility for service provision.
- HSE recognized First Aid at Work qualification or willingness to achieve one.
- Prior experience contributing to supervision or ability to demonstrate required skills.
- Knowledge of relevant legislation and guidance in relation to working with and safeguarding adults.
Salary and Benefits:
