Care Worker

We are seeking a skilled and compassionate Care Worker to join our team in Wiltshire Reablement. This is a fantastic opportunity to make a positive impact on people's lives, helping them regain independence and live safely at home.
The estimated salary for this role is £28,000-£35,000 per annum, depending on experience.
Key Responsibilities- Work closely with occupational therapy staff to create personalized care plans that meet customers' needs and ensure CQC compliance.
- Collaborate with partners, including health teams, care homes, and hospitals, to provide seamless support services.
- Conduct thorough risk assessments to identify eligibility for service provision and manage cases transferring into the reablement service.
- Ensure accurate record-keeping and documentation, maintaining high standards for CQC inspections.
- NVQ Level 4 or equivalent significant experience in a relevant field (or a commitment to pursue one).
- Excellent ICT skills, including Microsoft applications and specialist systems.
- Fully fluent in spoken and written English.
- Ability to work independently and prioritize tasks effectively against deadlines.
