About the Position

This senior role within our Reablement Service presents an exceptional opportunity for a dedicated and skilled individual to lead and support our team of Support Workers. As a Senior Support Worker, you will be responsible for overseeing the delivery of high-quality reablement services to customers in their homes.

Main Duties:

  • Supervising and mentoring Support Workers to ensure consistent high-quality service delivery.
  • Developing and implementing effective reablement support plans in collaboration with occupational therapy teams.
  • Conducting regular risk assessments and promoting a culture of positive risk-taking.
  • Coordinating with healthcare professionals and other stakeholders to ensure seamless care transitions.
  • Maintaining accurate records of customer interactions and progress.

A salary of £38,000 per annum is available, based on experience, plus a £650 welcome payment and a further £650 upon successful completion of probation. If you possess excellent leadership and communication skills, and a passion for empowering adults to live independently, we encourage you to apply.
