Complex Care Coordinator

- Deliver comprehensive mental health assessments and develop treatment plans that prioritize recovery and empowerment.
- Provide specialist therapeutic interventions, including crisis planning and risk management.
- Work collaboratively with service users, carers, and other healthcare professionals to ensure seamless care coordination.
- Maintain accurate records and reports, ensuring timely and effective communication with stakeholders.
- Participate in clinical supervision and contribute to the development of others through mentoring and training.
We offer a competitive salary range of £43,041 - £70,305 per annum, depending on experience, and a comprehensive benefits package, including pension scheme, generous annual leave, and professional development opportunities.
