Reablement and Care Coordinator

About the Job
As a Senior Support Worker at Wiltshire Council, you will be responsible for delivering high-quality support to customers, ensuring their safety and well-being at all times. You will work closely with occupational therapists to co-develop personalized reablement plans, providing crucial assistance to customers in the comfort of their own homes.
What We Offer
We offer a competitive salary of £25,000 - £30,000 per annum, dependent on experience, along with a £650 welcome payment upon starting and a further £650 upon successful completion of your probation (subject to tax and NI). Our team is committed to safeguarding and promoting the welfare of children, young people, and vulnerable adults, and to the principles of inclusion and diversity in both employment and the delivery of services.
Your Role
- To provide high-quality support to customers, ensuring their safety and well-being at all times.
- To collaborate with occupational therapists to develop and implement personalized reablement plans.
- To operate on a rotating schedule, working between 7 am and 10 pm, 365 days a year.
