About the Role

We are seeking an experienced Rural Community Healthcare Coordinator to join our team in North Powys, Wales. The successful candidate will have a proven track record of working with individuals who have frailty and be able to provide expert advice and specialist nursing knowledge.

Key Responsibilities
  1. Coordinate and provide essential support to patients living in rural and urban community settings.
  2. Conduct comprehensive geriatric assessments, risk assessments, and risk management plans to ensure the needs of patients are met.
  3. Develop and implement holistic care plans that aim to keep patients at home for longer, reduce acute admissions, and prevent hospital re-admission.
  4. Collaborate with Public Health Wales, Health Champions, and Primary Care to develop initiatives that improve health literacy, health outcomes, and enhance the experience of healthcare services for service users and their families.
Benefits and Salary
