Senior Frailty Nurse Specialist

We are seeking a highly skilled and experienced Senior Frailty Nurse Specialist to join our team in North Powys. This is an exciting opportunity to develop and lead a Frailty Nurse Service, working closely with patients, families, and carers to provide expert care and support.
The successful candidate will have significant experience of working with people who have frailty, post-registered experience working in community settings, and excellent consultation and liaison skills. They will be able to develop and deliver training plans for frailty nursing colleagues and have current knowledge of local and national drivers for community frailty services.
This is a 2-year fixed-term contract, and we offer a competitive salary of £32,000 - £45,000 per annum, depending on experience. We also prioritize work-life balance and well-being, recognizing the importance of achieving a healthy 'life-work' balance.
In this role, you will be responsible for coordinating and providing essential additional support to our most frail patients, supporting the development of expert frailty services in the community. You will work closely with a senior Band 8a nurse, supporting the development of the Frailty Nurse led services in North Powys.
