Advanced Community Frailty Nursing Lead

Job Overview
Powys Teaching Health Board is seeking a highly skilled and experienced Advanced Community Frailty Nursing Lead to join our team. As a key member of our Primary and Community Care services, you will be responsible for coordinating and providing essential support to some of our most vulnerable patients.
The ideal candidate will have a Master's level qualification or equivalent experience and possess excellent clinical assessment skills, with a person-centred approach to formulating comprehensive care plans. They will also have experience working with frail elderly patients in a community setting and be able to navigate the provision of care effectively.
Main Responsibilities
- Assist Primary and Community Care service leads in developing and ensuring robust delivery of Primary and Community Care Frailty Nursing services
- Provide clinical leadership and direction to Frailty Nursing services through clinical supervision and development of services
- Initiate planning, evaluation, and audit of practice, clinical pathways, and protocols for Primary and Community Care Frailty Nursing Services
- Ensure standards of Frailty Nursing services meet Clinical Governance requirements as specified by Powys Teaching Health Board
Required Skills and Qualifications
- Evidence of specialist knowledge and application of current best practice
- Ability to work under pressure and set priorities
- Experience of taking a clinical lead role within an appropriate setting
- Knowledge of current Nursing issues within NHS care systems
Salary and Benefits
