Do you have a passion for improving health outcomes and enhancing the experience of healthcare services for individuals with severe frailty? We're seeking an Advanced Practice Clinician to join our team at Powys Teaching Health Board.

We're offering a competitive salary of £35,000 - £40,000 per annum, dependent on experience. This is a fixed-term 2-year contract due to funding.

About the Service

The Frailty Nurse-led service provides expert advice and specialist nursing knowledge to individuals with severe frailty, their families, and carers. You'll work closely with the senior Band 8a nurse to develop the service in North Powys.

The ideal candidate will have significant experience working with people who have frailty and will be able to use their own initiative to make clinical decisions and provide clinical direction for the Primary Care and Community Frailty team.

You'll also be responsible for reviewing policies related to the Primary and Community Care Frailty Nurse service and developing training plans for frailty nursing colleagues.
