The Powys Teaching Health Board is seeking an exceptional Advanced Clinical Nurse Specialist to join our team. In this role, you will have the opportunity to make a meaningful impact on the lives of our patients and contribute to the development of expert frailty services in the community.

Job Description:

The successful candidate will be responsible for coordinating and providing essential additional support to some of our most vulnerable patients. This will involve thorough clinical assessment skills and a person-centred approach to formulate comprehensive care plans that aim to keep patients at home for longer, reduce acute admissions, and prevent hospital re-admissions.

Main Duties:
  • Assist the Primary and Community Care service leads in developing and ensuring robust and sustainable delivery of Primary and Community Care Frailty Nursing services.
  • Provide clinical leadership and direction to Frailty Nursing services through clinical supervision and development of services.
  • Initiate the planning, evaluation, and audit of practice, clinical pathways, and protocols for Primary and Community Care Frailty Nursing Services.

We offer a competitive salary of ££58,840-£67,684 per annum, depending on experience, as well as a range of benefits including a generous pension scheme, flexible working arrangements, and opportunities for professional development.
