About the Role:
This is an excellent opportunity to join our team as a Clinical Lead Nurse and make a real impact on the lives of our patients. As a key member of the primary and community care team, you will be responsible for leading the development and delivery of frailty nursing services in North Powys Cluster.

The successful candidate will have a strong background in nurse leadership and experience in managing complex care pathways. You will be able to communicate effectively with colleagues and stakeholders at all levels and have a proven track record of delivering high-quality patient care.

The salary for this role is approximately £60,000 per annum, commensurate with your experience and qualifications.

What You'll Do:
  • Lead the development and delivery of frailty nursing services in North Powys Cluster
  • Work closely with service leads to provide clinical leadership and direction to Frailty Nursing services
  • Manage complex care pathways and coordinate intra and inter professional projects
