Job Description:
We are seeking an experienced Community Mental Health Practitioner to join our team. As a key member of our community mental health team, you will be responsible for assessing the care needs of clients and developing personalized programs to support their well-being.

About the Role:
This is an exciting opportunity to work in a rural setting where you can make a real difference in the lives of our patients. You will be working closely with other healthcare professionals to deliver comprehensive care packages that meet the unique needs of each individual.

Key Responsibilities:
  • Assess care needs and develop, implement, and evaluate programs of care for clients in the community
  • Ensure good links with others within the designated Dementia, Community Mental Health Team area and other relevant agencies and professionals
  • Participate as an active member of the community team and assume the role of the key worker for patients
  • Take charge of the team caseload in the absence of the Team Leader

Requirements:
To be successful in this role, you will need to have a strong background in mental health nursing or social care, with experience of working in a similar capacity. You must also possess excellent assessment and planning skills, as well as the ability to communicate effectively with patients and colleagues alike.

Salary: £35,000 - £42,000 per annum (dependent on experience)
