The Powys Teaching Health Board is seeking a highly skilled Frailty Service Manager to join our team. In this role, you will be responsible for managing a small team providing care and support to those with complex medical and social issues.

Duties:
  • Coordinate and provide essential additional support to our most vulnerable patients.
  • Develop and implement comprehensive care plans to keep patients at home for longer, reduce acute admissions, and prevent hospital re-admissions.
  • Monitor the implementation of Standards for Health Services for Frailty Nursing across Powys North Cluster participating practices.

We offer a competitive salary of ££58,840-£67,684 per annum, depending on experience, as well as a range of benefits including a generous pension scheme, flexible working arrangements, and opportunities for professional development.
