Rural Community Healthcare Coordinator

We are looking for a highly skilled Frailty Service Development Lead to join our team in North Powys, Wales. The successful candidate will have significant experience working with individuals who have frailty and be able to provide expert advice and specialist nursing knowledge.
Main Accountabilities- Lead the development and implementation of Frailty Services in North Powys, working closely with the Frailty Team.
- Conduct comprehensive geriatric assessments, risk assessments, and risk management plans to ensure the needs of patients are met.
- Develop and implement holistic care plans that aim to keep patients at home for longer, reduce acute admissions, and prevent hospital re-admission.
- Collaborate with Public Health Wales, Health Champions, and Primary Care to develop initiatives that improve health literacy, health outcomes, and enhance the experience of healthcare services for service users and their families.
- Registered Nurse with current NMC registration.
- Relevant master's degree or equivalent level of knowledge, skills, and experience.
- Significant experience working with people who have frailty and developing training plans for frailty nursing colleagues.
- Excellent consultation and liaison skills, ability to develop and deliver presentations, and teach others.
