About the Role

We are seeking a highly organized and communicative Flow Coordinator to join our team at University Hospitals Birmingham NHS Foundation Trust. As a key member of our team, you will play a vital role in ensuring the smooth flow of patients through our Elderly Care Wards.

Key Responsibilities

  • Develop and maintain effective internal communication pathways to ensure seamless patient flow.
  • Utilize the internal escalation process to address any delays or issues that may arise.
  • Work closely with the multi-disciplinary team to ensure adherence to our internal professional standards.
  • Monitor the transport process and identify any potential bottlenecks.
  • Act as the first point of contact for users of the service, providing proactive support and ensuring that patients and their relatives/friends are welcomed and oriented to the clinical area.
  • Work collaboratively with staff at all levels to achieve our goals.
  • Deal with telephone enquiries professionally, ensuring that callers' needs are met or they are transferred to the most appropriate person.
  • Liaise with the Site Team as needed to ensure that all staff are aware of any information that will assist in safe transfers/discharge.
  • Promote all transfers/discharges within agreed timeframes.
  • Identify and address any system bottlenecks that may prevent the smooth and timely transfer of patients.
