About the Role

We are seeking a highly skilled and experienced Clinical Nurse Practitioner to join our Addictions Directorate within Central and North West London NHS Foundation Trust. The successful candidate will be responsible for providing specialist drug and alcohol outreach clinics for local hostels and homeless individuals, as well as organizing health services in the local authority catchment.

Key Responsibilities
  • Provide specialist drug and alcohol outreach clinics for local hostels and homeless individuals.
  • Work as a member of the multidisciplinary team in the management and treatment provision of a range of specialist services to service users experiencing difficulties with drugs and alcohol.
  • Promote health and wellbeing and improve health inequalities by completing a range of health checks and assessing service users' general health.
  • Develop and maintain pathways into specialist services to ensure the health and well-being of residents is met.
  • Manage a caseload of service users accessing rough sleepers support and ensure safety is maintained and risks are escalated appropriately.
Person Specification

Essential Qualifications

  • RMN (Registered Mental Health Nurse) or RGN (Registered General Nurse)
  • Evidence of continuing personal development and post-registration training in the nursing care, assessment, and treatment of substance misuse clients

Desirable Qualifications

  • Completed the Motivational Interviewing Training Module

Essential Experience

  • Experience working in Mental Health or substance misuse setting
  • Experience in undertaking Risk Assessment and Risk Management

Desirable Experience

  • Experience of working in and delivering services to injecting drug users
  • Experience of working with stimulant users and people with alcohol problems
  • Experience of group facilitation
  • Experience of managing a caseload within the community
  • Experience working with Patient Information Systems

Skills and Knowledge

  • Good communication and presentation skills (verbal and written) plus good levels of numeracy
  • Ability to work independently and to prioritize effectively
  • An appreciation of the range of issues that may affect substance misusers
  • An understanding of harm minimization and health promotion in the specific context of substance misuse
  • IT literate: competent using Microsoft Word and able to report clinical activity data on patient information systems
