Clinical Responsibilities

Core duties include, but are not limited to:

Patient Care

Deliver high-quality patient care as a Senior Clinical Nurse Practitioner in a general practice setting, utilizing advanced clinical skills and a comprehensive theoretical knowledge base.

Serve as the initial point of contact for patients presenting with undiagnosed conditions, employing skills in history taking, physical examination, problem-solving, and clinical decision-making to formulate a diagnosis and management strategy.

Order and interpret necessary diagnostic tests and evaluations.

As an independent prescriber, prescribe safe and effective medications in accordance with current regulations and local guidelines. Prioritize health issues and provide timely interventions, including emergency care.

Promote health and wellness through health education, screening, and effective communication strategies.

Provide up-to-date travel health guidance.

Clearly articulate patient health statuses using appropriate terminology and technology.

Ensure the delivery of safe, evidence-based, and individualized patient care.

Adhere to practice guidelines, policies, and protocols.

Maintain accurate and timely records, utilizing appropriate computer systems while considering data handling principles.

Administrative Duties

Support fellow healthcare team members and accept referrals as necessary.

Collaborate with all members of the primary care team and external agencies to ensure appropriate care for the patient population.

Actively participate as a key member of the multidisciplinary team, fostering collaborative and innovative practices.

Recognize the contributions of all team members.

Delegate appropriate tasks to nursing and healthcare assistant teams while maintaining overall responsibility.

Assist in implementing health and safety protocols as agreed within the practice.

Contribute to the development and maintenance of CQC guidelines within the practice.

Organizational Responsibilities

Complete necessary records, audits, and reports, responding to inquiries as needed.

Participate in the daily processing of clinical documents and lab results, preparing reports as requested.

Attend team and clinical meetings as required.

Support effective communication across the entire team.

Develop a thorough understanding of the appointment scheduling system and essential practice coverage needs.

Ensure supply orders remain within budget while actively seeking to minimize waste.

Contribute to practice strategy and development initiatives when called upon.

Adhere to clinical governance standards and support the clinical governance agenda.

Understand the importance of risk management within the practice, contributing to risk assessments and audits.

Participate in the assessment and management of patients in various settings.

Engage in achieving QOF targets and local enhanced service objectives.

Participate in teaching and mentoring activities within the practice.

Represent the practice at relevant meetings as requested by leadership.

Collaborate with the PCN and nursing forums.

Performance and Quality Improvement

Promote personal and professional development while striving for clinical excellence.

Work with the nursing team to develop new roles in response to evolving healthcare needs.

Facilitate and engage in multidisciplinary education and clinical supervision.

Adhere to the Nursing and Midwifery Council Code of Professional Conduct and Scope of Practice.

Participate in organizational decision-making, utilizing data to enhance practice.

Maintain professional development in accordance with Nursing and Midwifery Council regulations.

Advocate for evidence-based practice through research and guideline implementation.

Monitor the effectiveness of clinical practice through quality assurance strategies, including audits and peer reviews.

Engage in continuing professional development to ensure up-to-date knowledge and competence in all aspects of the role.

Stay informed about relevant health policies and collaborate with the practice team to assess their impact and implementation strategies.

Actively promote the role of the Senior Clinical Nurse Practitioner within the practice and the broader community.

Contribute to the development of practice improvement plans and new service offerings.
