Job Title: Advanced Clinical Practitioner Nurse

Job Summary:

We are seeking a highly skilled and experienced Advanced Clinical Practitioner Nurse to join our Primary Care Network team. The successful candidate will be responsible for delivering high-quality patient care and contributing to the development of our network's clinical services.

Key Responsibilities:

  • Provide advanced clinical practice to patients in our Primary Care Network, including health promotion, chronic disease management, and health prevention.
  • Work collaboratively with our multidisciplinary team to develop and deliver specialist primary care services, such as long-term disease management.
  • Contribute to the development of patient-centered strategies to address community health needs, in liaison with community health professionals and agencies.
  • Monitor the effectiveness of clinical practice through quality assurance strategies, such as audit and peer review.
  • Participate in continuing professional development opportunities to maintain up-to-date evidence-based knowledge and competence.
  • Work within the latest NMC Code of Professional Conduct and record accurate consultation data in patient records.
  • Proactively promote the role of the Advanced Nurse Practitioner within our network and externally to key stakeholders and agencies.
  • Help the network operate in a cost-effective manner and identify and manage nursing care risks.

Requirements:

  • 3 years of nursing experience, with a focus on primary care.
  • NMC registration and a master's degree in a relevant area of expertise.
  • Experience of working in a multidisciplinary team and contributing to the development of clinical services.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, colleagues, and external agencies.
