Advanced Clinical Practitioner Nurse

We are seeking an experienced and skilled Advanced Clinical Practitioner Nurse to join our team at North Lewisham Primary Care Network.
About the RoleThe successful candidate will be responsible for providing high-quality patient-centered care, working autonomously and as part of a multidisciplinary team to deliver clinical services and support the development of our Primary Care Network.
Key Responsibilities- Provide first point of contact for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision making, to establish a diagnosis and management plan.
- Make professionally autonomous decisions for which you are accountable.
- Instigate necessary invasive and non-invasive diagnostic tests or investigation and interpret findings/reports.
- Prescribe safe, effective and appropriate medication as defined by current legislative framework.
- Provide safe, evidence-based, cost-effective, individualized patient care.
- Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care.
- Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.
- Actively take a personalized care approach and population-centered care approach to enable shared decision making with the presenting person.
- Refer patients directly to other services within or outside the PCN, and agencies as appropriate.
- Participate in developing and delivering specialist primary care services such as long-term disease management (diabetes, asthma, chronic obstructive pulmonary disease, coronary heart disease and hypertension).
- Work with nursing, medical and healthcare assistant colleagues across the PCN to ensure that National Service Frameworks (Coronary Heart Disease/Older People/Diabetes/Mental Health) are being delivered.
- Work with the PCN multi-disciplinary teams as well as those within the PCN member GP practices and across the wider CIC, to promote integrated and seamless pathways of care.
- Contribute to the PCN achieving its quality targets to sustain the high standards of patient care and service delivery.
- Participate in identification of community health needs and develop patient/family-centered strategies to address them in liaison with appropriate community health professionals and agencies.
- Be aware of and contribute to, planning and delivery of practice-based commissioning.
- Current RGN Registration.
- Membership of a professional body RCN.
- Advanced Nurse Practitioner Degree or BSc in Emergency Care (or equivalent).
- Independent Non-Medical Prescriber Qualification (V300).
- Previous primary care experience.
- Experience of nurse-led minor illness and triage.

- Minimum of 2 years administration experience in any setting.
