We are seeking a highly skilled and experienced Senior Nurse Practitioner to join our team at Guy's and St Thomas' NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality, patient-centered care to individuals with palliative care needs in their own homes within the Boroughs of Lambeth and Southwark.
Key Responsibilities- Assess, plan, implement, and evaluate care delivered to patients with palliative care needs, ensuring a seamless provision of care.
- Manage a group of patients in collaboration with the Senior Nurse Practitioner, delivering individualized and personalized direct patient care overnight.
- Effectively hand over to relevant teams working in the daytime to ensure continuity of care.
- Provide specialist advice in the assessment, planning, implementation, and evaluation of patient care.
- Be responsible for the correct administration of prescribed medication, including management of syringe pumps, enteral nutrition, intravenous therapy, etc.
- Recognize and respond to palliative care emergencies in the specialty.
- Act as an advocate for patients, ensuring a patient-centered approach to the delivery of care and meeting standards within the NHS Plan and other relevant guidelines.
- Ensure patients receive high-quality clinical care and a good patient experience, having regard for their customs, culture, social circumstances, religious beliefs, and doctrines.
- RN1/RN2/RN12 on the NMC register.
- BSC/1st degree (health-related) or equivalent qualification/training and experience.
- Post-registration qualification in an appropriate area of specialty.
- Previous post-registration experience within the relevant specialty.
- Experienced and competent in the management of patients within the specialty.
- Experience of teaching/assessing/supervising others in a clinical setting.
- Supervision and support of junior staff.
- Demonstrates specialist expertise underpinned by theory acquired through CPD.
- Up-to-date knowledge of current clinical and professional issues, knowledge of evidence-based practice.
- Ability to assess, plan, implement, and evaluate care delivered to patients/clients and give relevant advice/information to patients and relatives.
- Evidence of involvement and leadership in teaching and mentoring learners.
- An understanding of audit and research-based nursing practice.
- An understanding of resource management.
- Good communication skills and ability to deal with sensitive information.
